These patients have no symptoms at rest, but during periods of maximum effort they experience pain or cramps and a loss of power associated with thigh or lower extremity. These symptoms are relieved when the effort is decreased or stopped. Claudication of the lower extremity may occur in high-strength athletes due to the fibrosis of the external illic artery, where the inner layer of the arterial wall thickens. This leads to a decrease in the diameter of the external iliac arterial lylium, causing a reduction in blood flow to the lower extremity during the maximum effort, leading to symptoms associated with the claudication. It may also be possible that the claudication associated with the external iliac artery endofibrosis is due to vasospasms of the blood vessel during the high intensity exercise that causes the decrease in blood flow. The endofibrosis of the external artery is a slow and progressive disease and it is unknown what causes the thickening of the intimal wall of the artery.
